Case Manager

Position Summary: The Case Manager is responsible for providing supportive Services including home-based visitations and office consultations at least once a month ensuring that Dignity Housing residents’ basic needs are met. The Case Manager is required to conduct basic needs assessments and work with the residents to develop a service plan delineating goals and action steps toward achieving housing stability and self-sufficiency. The Case Manager is responsible for monitoring progress, updating the service plan, and providing referrals and resources related to basic family needs, employment, education, housing, health and wellness. The Case Manager is required to keep detailed records and documentation of residents’ progress and status. The Case Manager is also required to work in conjunction with other Dignity Housing Staff and collaborative partners. Additionally, the Case Manager is responsible for working with a caseload of 16-24 residents. This position is full-time, 35 hours per week. Salary is $17-$18 per hour, depending on experience. There will be some evening and Saturday work required. Start Date March 1, 2017


Bachelor’s Degree required; 2 years experience preferred. Familiar with standard case management procedures, documentation and reporting; strong knowledge of government benefits and community resources within a social service environment; strong knowledge of domestice violence, addiction and mental health pathologies; proficiency in most general computer operation systems and applications required, including: Word, Excel, Outlook and Client Database. Excellent time management and organizational skills, excellent written and oral communication skills, highly energetic and creative; able to work in a team; passionate about improving the lives of others while maintaining appropriate boundaries.
